Community Care Liaison

As a Community Liaison youll contribute to our success in the following ways:

  • Creates and implements a strategic plan for territory growth and development.
  • Implements provider relations strategies to positively affect referral base and revenue in assigned territory.
  • Identifies and visits case managers discharge planners social workers physicians and any other potential and/or repeat referral sources on a scheduled and regular basis..
  • Documents efforts appropriately. Submits to RVP and/or AVP a planned schedule weekly contact reports indicating status and follow-up plans for contacts and quarterly provider relations analysis.
  • Maintains working knowledge of industry competitors as a continuous measure to ensure Elara Carings ability to retain and grow market share.
  • Maintains working knowledge of health care industry including legislative and regulatory changes which will impact business.
  • Collaborates with Leadership to plan implement and participate in trade shows and other opportunities to promote Elara Caring in the community for referral generation or field staff recruitment purposes.
  • Collaborates with Leadership in contract assessment preparation and negotiation as necessary.
  • Communicates new contract information to all office staff as applicable.

What is Required

  • Bachelors Degree in Business Administration Sales or related field
  • 5 years experience in healthcare sales
  • Ability to effectively communicate in writing and verbally
  • Ability to travel approximately 50% as business needs require.
  • Must have dependable vehicle valid drivers license current auto insurance in accordance with the laws of the state.
  • Previous experience in home health or hospice is preferred
