This role supports patient throughput and transitions of care through care coordination and utilization management in the Emergency Department.

Duties

Monitor patient flow and discharge readiness
Perform utilization reviews
Coordinate post-acute services
Collaborate with clinical teams
Maintain compliance documentation

Qualifications

NY RN license
Minimum 5 years RN experience
Care management experience required
ASN/ADN
BLS

Preferred

BSN/MSN
