Position Overview

Navitas Healthcare LLC is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join a Long-Term Care (LTC) setting in Youngsville PA. This role involves providing high-quality nursing care to residents in a skilled nursing facility while collaborating closely with Registered Nurses physicians and interdisciplinary care teams.

Responsibilities

Provide direct nursing care to residents in a long-term care environment
Administer medications and treatments in accordance with physician orders
Monitor assess and document residents conditions; promptly report any changes
Assist in developing and implementing resident care plans
Maintain accurate and timely medical records
Support residents with activities of daily living (ADLs) as needed
Ensure compliance with facility policies safety standards and state regulations
Communicate effectively with residents families and healthcare team members

Qualifications

Active Pennsylvania LPN license (required)
Minimum 1 year of LPN experience preferably in Long-Term Care or Skilled Nursing
BLS certification (required)
Strong clinical judgment organizational and documentation skills
Ability to work variable shifts and 12-hour schedules
Compassionate patient-centered approach to care
