Overview:
We are seeking a licensed Occupational Therapist (OT) to support patients in a Skilled Nursing Facility (SNF) and Long-Term Care (LTC) setting. This contract role offers a reliable daytime schedule with guaranteed hours and the opportunity to work with a collaborative rehabilitation team focused on improving patient independence and quality of life.

Schedule:

Day shifts
36 hours per week guaranteed
No holiday coverage required

Responsibilities:

Evaluate patients and develop individualized occupational therapy treatment plans
Deliver therapeutic interventions to improve functional mobility self-care and daily living skills
Monitor document and report patient progress in accordance with regulatory standards
Collaborate with interdisciplinary healthcare teams to ensure continuity of care
Maintain compliance with facility policies safety standards and professional guidelines

Practice Setting:

Skilled Nursing Facility (SNF)
Long-Term Care (LTC)

Requirements:

Active state Occupational Therapist license
Minimum of 1 year of experience in a SNF and/or LTC setting preferred
Strong clinical assessment documentation and communication skills
Ability to work independently and as part of a multidisciplinary team
