Job Summary

Job Title: Therapist Youth ACT
Location: Bay Shore NY

Job Description:

We are seeking a licensed or license-eligible Therapist to join our Youth Assertive Community Treatment (ACT) team. This community-based program serves youth ages 10 21 with serious behavioral health needs helping them remain safely in their homes and communities through intensive team-based support.

Responsibilities:

Operate within program policies procedures and agency practice guidelines.
Adhere to state regulations and applicable codes of conduct.
Collaborate within an interdisciplinary team to support client functioning and goal attainment.
Provide person-centered individualized family individual and group therapy in home community and office settings.
Deliver services in a trauma-informed culturally and linguistically competent manner.
Conduct comprehensive clinical assessments and develop individualized treatment plans.
Apply agency-approved evidence-based interventions.
Participate in daily team meetings family meetings supervision and required trainings.
Coordinate care with schools DSS and other collateral contacts.
Provide crisis intervention and participate in an on-call rotation outside normal business hours.
Assist with client transportation to service locations as needed.
Complete timely documentation reports and other program-related tasks

Time Breakdown:

60% Direct client contact (including transportation).
20% Documentation and case-related meetings.
20% Team meetings supervision and training.

Qualifications:

LMSW LCSW LMHC or LMFT (limited permits accepted).
Experience working with children/adolescents and families preferred.
Strong documentation and computer skills (EHR experience a plus).
Valid NYS drivers license and daily access to a personal vehicle.
