Position Title: Case Manager - Bilingual

Reports To: Division Director

Classification: Regular Full-Time (Non-Exempt)

Location: Washington DC - Northwest

Hours: Flexible schedule (Monday-Friday) between 7 am-8 pm Some evenings holidays and weekends may be required to support operations and respond to emergencies.

Summary: The La Casa PSH Program is a Permanent Supportive Housing program in the District which contains 40 studio-sized apartments in a state-of-the-art building. The program provides comprehensive case management services. The Permanent Supportive Housing program follows a Housing First model and philosophy and utilizes best practices to ensure participant success. The Case Manager provides direct case management services to previously chronically homeless single adult males living in single-site Permanent Supportive Housing. The Case Manager will assist participants in identifying and achieving immediate and long-term goals that will allow them to stabilize and remain in permanent housing. Case managers will work to empower participants in the housing program while promoting continued growth. The Case Manager will deliver participant-centered services to a caseload of 19 men who live on-site.


Responsibilities:

  • Provide supportive participant-centered trauma-informed case management services to recently homeless adults with serious mental illnesses addictions medical needs this Permanent Supportive Housing Program serving 40 single men.
  • Complete intake assessments and the DHS application with new participants and provide an orientation to the building and program.
  • Assist participants in developing community-based support networks.
  • Deliver participant-centered face-to-face contact to residents in both individual and group settings.
  • Foster wellness and independence with participants.
  • Engage participants and assess individual needs preferences and strengths.
  • Respond to immediate participant needs providing assistance in crisis situations resolving conflicts assisting in meeting basic needs.
  • Report incidents in accordance with agency policy.
  • Develop implement and revise individual service plans with each resident on caseload for both OCTO and HMIS database systems.
  • Document all contacts made with caseload into OCTO database system weekly and monthly into HMIS.
  • Provide comprehensive case notes that reflect case plan goal-oriented updates and status into database systems.
  • Maintain consumer electronic records by collecting data and completing clinically appropriate progress notes.
  • Monitor medical and psychiatric needs and assist caseload with medication management.
  • Collaborate professionally with external agencies providing services to participants within the restraints of HIPAA.
  • Collaborate through treatment team meetings weekly calls and regular follow-ups with Community Agencies.
  • Form and maintain positive relationships with the community and stakeholders.
  • Participate in multidisciplinary meetings and regular supervision.
  • Demonstrate empathy and flexibility in responding to participant needs and requests.
  • Attend agency trainings TCP trainings and conferences as needed or required.
  • Follow all agency policies and procedures.
  • Perform all other duties as assigned.
Requirements

Education:

  • Bachelors degree in human services or related field with 2 years experience in a similar setting;
  • Masters degree in human services field preferred.


Background & Qualifications:

  • Bilingual (Spanish/English)
  • Individuals with life experience with homelessness who meet other qualifications preferred
  • Prior relevant work experience in human or homeless services psychiatric rehabilitation additions poverty foster care mental health youth/young adults LGBTQIA people and/or other individuals from historically marginalized communities strongly preferred
  • Must have or be able to obtain a First Aid/CPR/AED certification that includes a hands-on skills testing module. Friendship Place will provide First Aid/CPR/AED training
  • Excellent time management detail-oriented organized capable of maintaining confidentiality and able to work independently with minimal supervision
  • Understanding and/or experience working with Veterans
  • Experience with HMIS data entry preferred
  • Must possess and be able to demonstrate strong intra- and interpersonal communication skills with the ability to effectively express ideas needs and observations both verbally and in writing. This includes the ability to communicate clearly and respectfully with clients colleagues and supervisors as well as to document interactions and incidents accurately professionally and in a timely manner
  • Must be able to demonstrate active listening emotional awareness and the ability to adapt their communication style to different individuals and situations

  • Proficiency in Office 365 system specifically Word and Excel

Working conditions: The office is located in an apartment building and exposed to conditions that arise in homes including occasionally biohazards (e.g. blood tissue human waste) and conflict.


Physical and Environmental Requirements

The position will spend most of the time moving through the program site providing supportive services to residents and desk settings for required reporting. Employees must be able to sit and stand for extended periods of time with repetitive motions and positions. Some lifting may be required (up to 20-25lbs).


Background Checks: This position is classified as Safety Sensitive under DHS. The selected candidate will be required to complete and pass background checks specific to contract requirements that may include criminal history background check and fingerprinting FBI Background Check DC Metropolitan Police Background Check State-of-Residence Police Background Check DC Child Abuse Registry (CFSA) National Sex Offender Registry Check Drug Panel and Alcohol Screening Tuberculosis Test and Driving Record check (for positions transporting participants). Reference checks and verification of current and/or previous employment are required.

Specific Requirements: The employee must have a valid government-issued ID and have reliable transportation to arrive at the worksite on time.

Compensation: The hiring pay range is between $58117.00 - $63506.00 annually. Depending on the position the starting hourly pay rate or annual salary is based on a step system. A candidates starting pay determination is made based on meeting the basic position requirements and current completed education credentials and/or work experience. Please Note: The step system is set and not open to pay negotiation. Before moving forward in the selection process interested applicants are encouraged to discuss starting pay eligibility with the Human Resources (Talent) Department.
