Advanced Practice Provider Hematology Medicine Fellow

TheAdvanced PracticeProvider (APP)Fellowisprovidedspecialized training in the management ofassigned specialtypatients in a variety of settings to ensure optimal quality of care and gaining necessary skill sets to develop and strengthen proficiencies within the clinical setting while learning to collaborate within a multidisciplinary team of healthcare providers.TheAPP- Fellowperformspatient assessment to include medical history and physical exam in collaboration with the physician/preceptordevelopsan appropriate plan of care in the direct management of patients with acute and chronic illnesses.The fellowdisplayscompetencieswithpreceptorand collaborating physicians.Works directly with staff administration patientsand families to ensure quality care across the continuumto evolve as anexpert in selected practice areas.Integrates current knowledge of professional issues trends in health careand technological advances intoscopeof practice.Participates in the collections of research data quality improvement discharge planningand coordination of patient care within the acute care areas.

TheAdvanced Practice Provider Fellowshipprogram at the University of Miami Miller School of Medicine will complete rotations in a variety of clinical settinglocatedwithinUHealthto include both inpatient and outpatient settings over a period ofweeks as assigned by specialty.

CORE JOB FUNCTIONS

Clinical Role

• Provides care to a select group of patients bydeterminingandrenderingadvanced nursing and medical interventions.

• Assesses patients by interviewingandexamining the patient reviewing the patients medical history physicians diagnosis and orders diagnostic test results (labs x-ray MRI etc.) todeterminemedical diagnosis plan of careandmonitorpatients progress.

• Reviewsmedical recordsincluding physician/consultation notes labsand diagnostictestsresultstoformulate medical diagnoses.

• Collaborates withphysicianand preceptorsthrough discussion and assessment of diagnosesto develop a plan of careandidentifiesexpected outcomes including interventions.Documents and coordinates plan of care.

• Provides patient/family educationregardingdisease processes treatment options treatment outcomespossible complications medications discharge planningand lifestyle modification.

• Integrates current knowledge of professional issues trends in health careand technological advances intoscopeof practice.

• Collaborates with team members to ensure implementation of planandemploysphysician orders perprotocols.

• Assistsin the management of family dynamics and coping mechanisms during acute and chronic phases of patient care.

• Evaluates the patients progress inattainingexpected outcomes byfollow-upwithcollaboratingphysician.

• Collaborates with members of the education department and units leadership team in planning educational opportunities and competency evaluation for nursing staff members.

• Conducts orparticipatesin multidisciplinary rounds and ensuresappropriateand quality care for patients.

• Able to progress from observer to an independently practicing APP with the guidance of their preceptor/physician collaborator

• Prioritize time to be able to manage clinical rotations study time and fellowship requirements

• Cross cover units/clinics independently when directed by Director Fellowshipprogramand preceptor input on readiness

Research/Quality Improvement

• Initiates and/orparticipatesin unit and/or hospital research activities. Utilizes research findingsand nursing theoriesin clinical practice. Promotes evidence/knowledge-basednursingpractice. Collects data for patient and program evaluation andparticipatesin quality assessment and improvement activities.

• Authors or contributes to an article for publication in a professional journal or book.

Professional Development

• Participates in 360 peer review process

• Submits annualprotocolsrevision to state nursing and medical boards

• Maintains national certification

• Specialtycertification upon completion of fellowshipif applicable

• Complete all competencies and training specified for allAPPsworking atUHealth

• Encouraged to attendto one national conference

• Presentation of clinical case studies at completion of each clinical specialty rotation

• Attendance to weekly educational grand roundsdisease specific team meetings andeducational sessions provided by department ofNursingand Advanced Practice

Supervision Received

• Reports tocollaborating physicians during clinical rotations andAPPMentor (assigned) and reports directly toleaderas assigned.

• Meets periodically with the Director Fellowship Program

• Adheres to University and unit-level policies and procedures and safeguards University assets.

Fellowship Program Requirements

• Completed an oral presentation on the topic of their choice

• Attempts tosubmitan article for publication

• Initiates and completes a quality improvement project in collaboration with department/unit leaders

• Sits for specialty certification if applicable to their area of specialty and licensure

Education:

• Mastersdegree inrelevant field

Certification and Licensing:

• Valid State of Florida APRN/PALicense; BLS/ACLS Certifications
