Role responsibilities center on safe patientcentered care for residents with complex needs. Youll perform thorough nursing assessments administer medications and therapies monitor vital signs manage wounds and implement care plans designed to prevent complications and promote functional independence. Youll collaborate with physicians therapists social workers and family members to coordinate care document clearly in the electronic health record and participate in rounds and care conferences. Youll lead or support activities of daily living promote fall prevention and infection control and be prepared to provide palliative and endoflife support with sensitivity and professionalism. The position offers substantial opportunities for growth within the LTC specialty including enhanced competencies in dementia care IV therapy wound management pain control and care coordination.
