Job Summary
Since 1980 Tidewell Hospice has proudly served families across southwest Florida with compassionate dignified care. As a member of Empath Health we help patients with advanced illness live meaningfully while supporting their families with warmth expertise and respect.
Schedule: Saturday & Sunday 8am-8pm and 1 weekday evening
Location: Primarily dedicated to the North but can be asked to visit any of our four service areas.
The RN Triage/Field:
- Visits patients and provides clinical physical educational and emotional support after-hours as directed by the Triage team (in in the patient home long term care facility assisted living facility hospice house or hospital)
- Under the direction of a licensed physician performs patient assessment manages discomfort and provides symptom relief using specialized nursing skills related to palliative and end-of-life care.
- Utilizes nursing interventions and teaching skills to maximize patient comfort and to enhance the quality of life to the patient/family unit.
- Attends patient deaths provides bereavement support to the patient caregiver/ family as indicated and performs post mortem care.
- Provides accurate and timely documentation of patient services within electronic medical record to reflect the individualized plan of care and outcomes of interventions.
- Reviews and updates the Plan of Care physician orders MAR/Pharmacy profile and other records to ensure patient/family needs are met.

What Youll Need
Current unrestricted Florida Licensure as RN.
BSN or MSN degree preferred.
Minimum one year recent acute care experience in a Hospital or Home Health environment; previous experience as a Hospice RN preferred.
CHPN certification is strongly encouraged within 3 years of employment.
An equivalent combination of professional experience and education may be considered as meeting the educational/professional requirements.
