We are seeking experienced Advanced Practice Providers (NPs & PAs) to join our Washington this role you arent just a provider; you are a mobile clinician equipped with the time technology and autonomy to make a real difference. You will manage complex acute episodes (CHF exacerbations infections post-op complications) with the support of a multi-disciplinary care team.

What You Will Do
- Respond & Assess: Travel to patient homes to perform problem-focused acute care assessments and screenings.
- Diagnose & Treat: Order and interpret diagnostic tests (Labs EKG POCUS) and administer treatments (IV fluids meds wound care).
- Coordinate: Close the loop with primary care providers and specialists to ensure continuity of care.
- Educate: Empower patients and families to manage their conditions and avoid the hospital.
What You Bring
- Licensure: Active unrestricted ARNP or PA-C license in the state of Washington.
- Experience: Minimum 2 years (or 3600 hours) of clinical experience in a high-acuity setting (ER ICU Urgent Care or Hospitalist).
- Certifications: Board Certification (NP-C/PA-C) and current BLS (ACLS preferred).
- Field Readiness: Valid drivers license ability to carry equipment (up to 60 lbs) and a comfort level with navigating non-clinical environments.
