Your Role As theCommunity Services Coordinator you will be responsible for the development implementation and facilitation of support services to Sertoma Consumers within the Home-Based and Day Services programs. This may include Self-Direction Assistance services Community Day Services and/or coordination of other various services in combination as chosen by the consumer. This position is a hybrid remote/office position and requires a flexible schedule (i.e. days evenings and weekends as needed). Responsibilities - Develop and implement Implementation Strategy for consumers; assist consumers with goals outcomes and objectives as stated in their Person-Centered Plan (PCP).
- Participate in the Discovery Tool process complete required annual assessments as necessary prior to the scheduled annual meeting in order to determine strengths abilities needs interests preference and choice in service provision.
- Work cooperatively with and provide direction and training to staff consumers guardians families students and volunteers as needed for implementation strategy data collection implementation strategy revision and to ensure appropriate access to services as identified in PCP. Responsible for monitoring and documentation of Implementation Strategy goals.
- Responsible for management and audit of all required consumer file documentation that is assigned (electronic records/paper records.) Also for agency management reports billing requirements and entry and other documentation required for compliance with Federal State and Agency policy & procedure.
- Ensure delivery and/or coordination of all services in compliance with DHS Rule 117 119 & 120 CARF standards agency mission agency policy & procedure program guidelines and best practice.
- For SDA services ensure and monitor provider compliance of Service Agreements & Authorizations through home and site visits and contact via telephone and virtual platforms. Ensure monitoring activity complies with individual service agreements and Medicaid requirements.
- For SDA services help guide individuals and families in arranging for directing and managing Personal Support Worker (PSW) services. Provide skills training designed to help individuals and families better recruit and hire PSWs as well as provide information on effective communication and problem-solving skills.
- For SDA services complete update and audit all required Sertoma documentation service agreements Medicaid waiver and data for assigned HBS consumers to ensure compliance with monthly allowable maximum budgets and adequate record keeping with funding source and agency requirements.
Qualifications - Bachelors degree in human services field of study required (Alternative Bachelors Degree with 30 semester hours in human services areas approved by DHS for QIDP Certification).
- Certification by DHSA as a qualified QID or the ability to complete training and obtain certification within 6 months.
- Minimum of one year experience working with individuals with developmental disabilities and/or mental illness preferred or the ability to reach one year of working experience within 6 months.
- Effective communication skills both oral and written.
- Valid drivers license and documentation of current auto insurance with an acceptable motor vehicle report.
- Proficient in the use of computers software applications and working knowledge of Microsoft Office Suite programs.
