Position Summary
Reporting to the Manager Patient Flow the Quinte Health at Home Coordinator is responsible for the effective management of processes and resources as they relate to the Quinte Health at Home Program. This would include identifying appropriate patients for bundled care ensuring referrals are appropriate from primary care or HCCSS supporting effective discharge planning with interprofessional teams and acting as a liaison between the home and community care provider and Quinte Health hospital care team including physicians. This role is accountable for contributing to the overall Quinte Health strategy and actively promoting and supporting patient and family centred care. This position may be working Monday to Sunday.
Required
Degree in a regulated health profession (BScN BScPT BScOT MSW MScSP)
Current member of applicable regulatory College in good standing
1 year experience in last 5 years liaising with community partners or coordinating hospital bases home care
Knowledge of community and government resources and associated legislation
Exceptional assessment negotiation and problem-solving skills
Exceptional interpersonal communication organization and time management skills
Ability to build and maintain relationships with hospital staff and external stakeholders and manage conflicting priorities
Ability to work in a fast-paced environment
Ability to work independently and collaboratively with a team
Effectively works within a team across multiple programs
Ability to wear personal protective equipment effectively as required
Demonstrated commitment to customer service and teamwork
Supportive of a practice model that encourages accountability communication value for money mutual respect and cooperation
Proficiency with computer and software is essential
Demonstrated attention to detail with documentation data collection and stakeholder reporting
Duties
Receive referrals (from ED inpatient primary care) and determine completeness eligibility and appropriateness for program
Liaise with hospital staff physicians and primary care to obtain appropriate information for referrals
Engage with patients and families to develop plans for transition
Act as a program ambassador and educate patients families and staff on the program
Work collaboratively with the home and community service provider CCS and other stakeholders to effectively transition patients
Collaborate with home and community partners and external stakeholders to identify plan and implement strategies to enhance patient flow
Be a driver for improving uptake of home to wait philosophy and facilitate timely discharge from acute or post-acute care to home
Lead daily care huddles with community partners
Follow up on program patients and progress towards identified goals
Complete home visits as required
Ensure effective transition plans are in place prior to discharge form the bundled program
Recording required data analysis of data and reporting any trends
Identify and report any opportunities for program improvement
