What Youll Do

As a Home Health LPN youll provide skilled compassionate care while helping patients remain safe and independent at home:

• Deliver nursing care based on the Plan of Care under RN and physician direction

• Provide patient-centered education and support for self-care

• Monitor document and communicate changes in patient condition

• Collaborate closely with RN Case Managers and interdisciplinary teams

• Promote patient safety and help prevent unnecessary hospitalizations or ER visits

• Build trusting relationships with patients families and caregivers

• Accurately document visits medications and outcomes

• Uphold the highest standards of professionalism and compassion

Job Requirements:

• Graduate of an accredited nursing program (Certificate Diploma or Associate)

• Current State LPN/LVN License

• 1 year of clinical experience (home health or hospice preferred)

• Comfort with independent field-based work

• Valid drivers license reliable transportation and insurance

• Sit stand bend lift and move intermittently and be able to lift 50-100 lbs

• Ability to travel (approx. 50%) and meet physical job requirements
