JOB DESCRIPTION


Summary/ Objective
East Side House Settlement (ESH) is a community resource in the Bronx and Northern Manhattan. ESHs Adult & Workforce Programs align with our mission by providing high quality programs and services within community centers schools and business partners.

Under the supervision of the Director of PSP and the PSP Employment Coordinator PSP Case Managers play a key role in the PSP staff inter-disciplinary team whose purpose is to provide comprehensive educational and social emotional support that enable young adults to meet their personal educational and employment goals. The primary focus for the PSP Case Manager is to support PSP participants in choosing a post-secondary track and completing outlined track deliverables for certification training professional soft skills internship hours and job placement. The Case Manager will communicate effectively with the Director of PSP and PSP Employment Coordinator to facilitate student intake case management and all other wrap around services needed for students who are enrolled in GED/ALE PSP Academy CRED and various other PSP service areas.

Essential Functionsand Responsibilities:

Under the supervision of theEmploymentCoordinator theCase Managerwillbe responsible forbut not limited to the following:

  • Caseload & Engagement:Manage a caseload ofapproximately 50100 participants per year (organized by cohorts);maintainregular contact at all phases to support progress engagement and retention.
  • Intake & Planning:Conduct participant intake needs/barrier assessments and develop individualized service or goal plans; provide regularfollow-upsand adjust plans based on progress and emerging needs.
  • Attendance & Retention:Conduct daily/ongoing outreachregardingattendance and engagement; use proactive strategies to reduce barriers and improve persistence.
  • InterdisciplinaryCoordination:Collaborate with InstructorsRetentionSpecialist and Education/Employment Services Managers to deliver coordinatedco-locatedservices and ensure participant goals are met.
  • Counseling & Coaching:Provideone-on-oneandsmall groupcounseling/coaching that supports academic career and personal goals; delivertimelyfeedback on progress and next steps.
  • Workshops:Facilitateworkforce readiness and basiclife skillsworkshops aligned to participant needs and trainingforinstructor credentials in CPR-First Aid and other competencies.
  • Referrals & Navigation:Connect participants to services across the local workforce development system (e.g. Career Centers NYS Department of Labor ACCESVR NYS Commission for the BlindTANFfundedsupports) and to community resources addressing housing food security legal aid and benefits.
  • Recruitment & Onboarding:Support recruitment and interviewing for new cohorts;maintainaccurateparticipant files and recordsin accordance withfunder and organizational standards.
  • Data & Compliance:Build andmaintaina shared database (e.g. Google Sheets/Drive/Salesforce) for caseload tracking; enter thoroughtimelycase notes; prepare reports andparticipateindata drivencase conferences.
  • Meetings & Development:Participatein team/department meetingstraining courses case conferences workshops and on/offsite events; contribute to continuous improvement of PSP services.
  • Other Duties:Duties responsibilities and activities may change at any time with or without notice.

Expected Work Schedule

This role is a full-time position (35 hours) expected togenerally workthrough Monday through Friday 9:00am to 5:00pm and 11:00am to 7:00pm some days. Some weekends and evenings may berequired. Workdays/hours subject to change based on ESHs needs.

Qualifications

Qualifications

To perform this position successfully theCase Managermust be able to fulfill all the elements of the job description to the highest standard and be able to work well with colleagues in a high energy and collaborative work include:

Required Education

degree in social workor a related field and experience working with City systems (e.g. justice publicassistance child welfare education housing); or

atleast three (3) years of successful experience providing case management/counseling and working with City systems.

Required Qualifications

  • Experience capturing andmaintainingdata and documentation in organizational/funder databases.
  • Demonstrated ability to form strong professional relationships with diverse participant populations and community stakeholders.
  • Excellent organizational verbal and written communication skills; strong initiativeproblem-solving and customerservice orientation.
  • Ability to work collaboratively on an interdisciplinary team; flexibility and poise under pressure.
  • TechnologyProficiency:
  • Microsoft Office (e.g. Word Teams)

  • Alsocomfortablelearning new platforms/systems.
