Job Summary

Summary

This position is at the Butler VA Medical Center within the Post-9/11 Military2VA (M2VA) Program as the RN Case Manager. The M2VA RN Case Manager provides clinical case management for severely-injured and/or ill post 9/11 service members and polytrauma Veterans with identified case management needs. Serves as an advocate for post 9/11 service members and Veterans and their families and caregivers helping them access needed services at the facility at other VA facilities and in the community


Duties

Major Duties include but are not limited to:

  • M2VA RN Case Manager is responsible for acquiring and demonstrating basic knowledge of health care programs to improve the quality of patient care the use of resources and to facilitate compliance with internal and external requirements and standards
  • Must have basic knowledge of medical and mental health diagnoses (included but not limited to amputation burns vision loss traumatic brain injury Post Traumatic Stress Disorder and the Adjustment/Anxiety/Mood disorders)
  • Works closely with a preceptor with a goal of developing skills and judgment in the areas of interpersonal relations critical thinking problem solving and conflict resolution
  • Collaborating with other case managers and other clinical and administrative staff as needed to ensure patient care needs are met.
  • Uses the nursing process in collaboration with the interdisciplinary team members.
  • Demonstration of the knowledge and skills necessary to provide care appropriate to the ethnic and cultural diversity as well as the age-specific challenges of the post 9/11 population (This knowledge includes understanding changes associated with aging and principles of growth and development relevant to the young adult adult and geriatric populations)
  • Serves as an advocate for post 9/11 service members and Veterans and their families and caregivers helping them access needed services at the facility at other VA facilities and in the community
  • Develops and utilizes community resources
  • Ensures Veteran and their significant others can identify who their M2VA Case Manager or Lead Coordinator is
  • Accountable and responsible for possessing the knowledge and skills to: 1. Communicate and interact appropriately and courteously with all internal and external customers. 2. Maintain confidentiality of electronic written and/or verbal patient/employee information
  • Begins the case management process prior to admission to the facility and works collaboratively with staff from other Veterans Health Administration (VHA) facilities Military Treatment Facilities and various referring facilities
  • Completes an in-depth assessment of functional status acuity level prognosis and assesses the need for treatment services and resources
  • Initiates contact with Veterans their families and caregivers to assist and ensure that the transition to the VHA is safe and continuous
  • Monitors patient status while the patient is receiving care at the facility
  • Develops a biopsychosocial Comprehensive Plan (CP) of care for post 9/11 inpatients and outpatients and may serve as the Lead Coordinator
  • Documents appropriately in the electronic health record Care Management Tracking and Referral Application (CMTRA)
  • The incumbent in collaboration with the M2VA Program Manager is responsible for coordinating performance improvement initiatives and for collecting relevant data required for Commission on Accreditation of Rehabilitation Facilities (CARF) certification and Joint Commission on the Accreditation of Healthcare Organizations (JC) standards as well as the Polytrauma Program
This clinical position includes a level of physicality normally associated with direct patient care duties performed by nurses such as potentially long periods of continued walking standing sitting moderate lifting repeated bending or kneeling. Examples may include but are not limited to: - Significant amounts of time standing assisting assigned patients. - Utilization of safe patient handling and mobility protocols.
