Job Description:

Position Purpose summary:

The Caseworker New Roots Transitional Housing is responsible for sourcing appropriate housing options for Participants to transition to independent living and provide wrap around support to the individuals post transition into community. The Caseworker must ensure that housing options comply with Federal/ Provincial law and Salvation Army policies and procedures reflecting and satisfying the preferences/circumstances of each participant.

Accountabilities:

Duties:

  • Develop implement and coordinate the case management process.
  • Gather information from assessments and Participants meetings to identify barriers to housing; develop and implement a Participant care plan outlining goals and steps taken to achieve them.
  • Help Participants to identify goals continually monitor and evaluate these goals and implement discharge plans for Participants that have obtained housing.
  • Accurately records day-to-day happenings using appropriate methods; maintains case notes and communicates appropriate information with other staff as needed.
  • Handles the orientation evaluation of Participants understanding of rules and discharge procedures; ensures the ongoing enforcement of policies ensures residents participation in the program.
  • Completes all reports on time and with appropriate detail for internal and external stakeholders/funders/evaluators.
  • Conduct referrals and coordinate with relevant community supports engaged in the Participants care plan; assisting the Participant to identify and obtain resources and supports.
  • Works with casework team and Manager to create/monitor and revise individual Participant programming including interventions in crisis situations.
  • Schedule presentations and workshops from outside organizations to support program need.
  • Facilitate Participant programming as required.
  • Supports staff and Manager with Participant room checks as required.
  • Create intake and discharge plans with Participants for submission and agreement with Manager.
  • Supports Participants in developing inter-personal relationships that promote dignity and respect.
  • Liaise with landlords housing partners external agencies families and other supports on behalf of the participants recording all such interactions into each case file.
  • Attend appointments with Participants as needed.
  • Initiate and/ or participate in case conferences as a forum for problem solving and exploring alternative solutions to service provision as required for individualized interventions and follow through on any action plans and monitoring required.
  • On discharge into appropriate housing support Participants daily reducing to no direct support as each Participants situation stabilizes. Each interaction to recorded in the individuals case file.
  • On discharge into appropriate housing support landlords or housing providers on an on-call basis to ensure continuance of housing for the Participant. Each interaction to be recorded in the individuals case file.
  • Maintain any mandatory training or certification.
  • Ensure safety practices in compliance with health and safety standards and prepare submit and act on incident reports; provide calming intervention as required to ensure the safety of everyone.
  • Responsible to report any serious infractions or problems in accordance with policies.
  • Undertake other duties as required relevant to the work of the Salvation Army

FINANCIAL AND MATERIALS MANAGEMENT:

  • Orders food and supplies from some store accounts
  • Participant purchases in consultation with and approval by the Manager

WORKING CONDITIONS:

  • Work environment is in the office at New Roots Transitional Housing and in the community of Prince Edward Island.
  • Some disagreeable conditions include handling garbage supporting cleaning odours dealing with angry people verbal abuse stress caused by large responsibility level.
  • Requires regular use of computer.
  • Requires being able to lift up to 20 pounds.
  • Requires considerable mental effort.

Education Qualifications and Certifications:

  • Completed an Undergraduate University Degree in a Social Service program or equivalent.
  • Completed Criminal Record Check with Vulnerable Screening
  • Safe Food Handling and First Aid/CPR Certifications are required.
  • Non-violent crisis intervention training would be an asset.

Experience and Skilled Knowledge Requirements:

  • Minimum of three years of prior related experience including experience in specialized social service programs supervision/management and working with marginalized clientele.

Skills and Capabilities:

  • Develop an understanding of the Mission of The Salvation Army in Canada and its implications as related to position responsibilities.
  • Represent the organization in a professional and engaging manner.
  • Ensure compliance with policies and procedures as it relates to external funders.
  • Responsible to work in compliance with the Prince Edward Island Health & Safety Act and Regulations
  • Treat the property of The Salvation Army with due care and caution.
  • Lead by example demonstrate a positive attitude a strong work ethic and a willingness to learn and be flexible in the face of change.
  • Knowledge of community-based resources and experience networking with government and social service agencies.
  • Have background in or working awareness of harm reduction trauma-informed care suicide intervention crisis intervention motivational interviewing.
  • Demonstrate an understanding of the psychological and social dynamics of working with individuals who have diverse needs and interests.
  • Strong organizational skills with the ability to complete assignments and meet deadlines with minimal supervision.
  • Proficiency in the use of computer programs such as Word processing Spreadsheets Databases E-mail and Internet etc.
  • Work cooperatively and effectively with others to set goals resolve problems and make decisions that enhance organizational effectiveness.
  • Valid PE Class G Driver license a current drivers abstract that is satisfactory to The Salvation Army in its sole discretion is required.
  • Screening through The Salvation Army Abuse Registry.
  • Ability and willingness to undergo applicable screening and background checks successfully that are satisfactory to The Salvation Army in its sole discretion.
  • NOTE: An alternative level of education and experience may be considered.

Compensation:

The target hiring range for this position is $19.67 to $24.59 with a maximum of $29.51.

Placement in the salary range will be based on factors such as market conditions internal equity and candidate experience skills and qualifications relevant to the role.
