As a Hospice RN Case Manager with Holy Cross Home Care & Hospice youll use advanced technology strong clinical judgment and a patientcentered approach to deliver exceptional oneonone care. Minimum Qualifications
Graduate of an accredited or stateapproved practical nursing program
Active LPN license in the State of Ohio
Minimum of one (1) year of acute care experience (home care experience preferred)
Phlebotomy and wound care experience required
Successful completion of the NAPNES medication exam or equivalent prior to medication administration
Minimum Qualifications:
Graduate of an approved nursing education program
Active RN license in the State of Maryland
1 years of clinical nursing experience (Hospice preferred)
Strong communication assessment and organizational skills
Compassionate dependable and mission-minded
Must have current Drivers license and reliable transportation

Pay Range: $36.70-58.71 per hour
