Job Summary

Summary

The Registered Nurse - Telehealth Clinical Technician (RN-TCT) is involved with the ongoing assessment monitoring and trending and case management of Veterans in their residential environment (or their environment of choice) and provides appropriate information to providers and the healthcare system to enable just in time care. The RN-TCT serves as case manager to utilize Home Telehealth (HT) technology in their practice of care coordination.


Duties

Duties include but not limited to:

  • Executes position responsibilities that demonstrate leadership experience and creative approaches to the management of complex patient care.
  • Care will be provided in the least restrictive most cost-effective manner.
  • Apply care and case management principles to coordinate care using health informatics disease management and telehealth technologies to facilitate access to care and to improve the health of Veterans with specific intent of providing the right care in the right place at the right time.
  • Improves clinical outcomes and access to care while reducing complications hospitalizations and clinic or emergency room visits for Veterans in post-acute care settings and high-risk patients with chronic and mental health disease.
  • Provides initial and ongoing comprehensive assessment to include a review of systems which establishes a comprehensive plan of care for Remote Patient Monitoring - Home Telehealth (RPM-HT).
  • Demonstrates the ability to plan and organize assignments independently and the independent technical knowledge and skills that are complex in nature that contribute to the Veterans treatment plan and support the successful completion of the Telehealth visit through the various Telehealth modalities.
  • Guided by policy and Standard Operating Procedures
  • Responsible for the patient site facilitation of the clinical business scheduling and technical aspects for real-time Telehealth applications.
  • Reads analyzes and interprets directives policies and regulations pertaining to telehealth services making recommendation of actions needed to ensure compliance with requirements.
  • Subject matter expert and maintains certification for Clinical Video-Conferencing Technologies (CVT) and Store and Forward Telehealth (SFT) processes.
  • Trained to perform a range of direct care duties that may include collecting vital signs phlebotomy lab specimen collection and other clinical components of a primary care visit as directed by policy or clinic standards.
  • Coordinates the scheduling of their telehealth clinics or visits.
  • Makes reminder phone calls to patients and providers as necessary.

    Basic Requirements:

    NOTE: Grandfathering Provision - All persons currently employed in VHA in 0610 series and performing the duties as described in the qualification standard on the effective date of the standard (1/29/2024) are considered to have met all qualification requirements for the grade held including positive education and licensure/certification.

Self-directed in goal setting for managing complex client situations.An ADN or Diploma in Nursing AND 2 years of professional nursing experience in which one year is equivalent to Nurse I Level 2; OR a BSN and 1 year of professional nursing experience equivalent to the Nurse I Level 2; OR a Masters degree in nursing (MSN) and no additional professional nursing experience; OR a Masters degree in a *related field with a BSN and no additional professional nursing experience.
    Nurse IIDemonstrates leadership in delivering and improving holistic care through collaborative strategies with others.A BSN with 2 years of professional nursing equivalent to Nurse I Level 3; OR an MSN with one year of specialized nursing experience equivalent to Nurse I Level 3; OR a Masters degree in a *related field with a BSN and one year of specialized nursing experience equivalent to Nurse I Level 3; OR a Doctoral degree in Nursing with no professional nursing experience; OR a Doctoral degree in a *related field with a BSN with no additional professional nursing experience.
    Nurse IIIExecutes position responsibilities that demonstrate leadership experience and creative approaches to management of complex client care beyond the immediate practice setting.MSN and 2 years of specialized nursing experience one of which is equivalent to Nurse II and meets all dimension requirements for Nurse III; OR a Masters degree in *related field with BSN and two years of specialized nursing experience one of which is equivalent to Nurse II and meets all dimension requirements for Nurse III; OR a Doctoral degree in Nursing with and one year of specialized nursing experience equivalent to Nurse II and meets all dimension requirements for Nurse III; OR a Doctoral degree in a *related field with a BSN and one year of specialized nursing experience equivalent to Nurse II and meets all dimension requirements for Nurse III.
    *Note: Foreign education programs/degrees are not creditable as related degrees.

    Physical Requirements:
    This position requires visual acuity keen hearing clear distinctive speech and manual dexterity. This position requires the nurse to read printed material and distinguish colors such as reading doctors orders and visualizing the hue of a patients skin wounds etc. and conduct visual inspections during patient assessment including the ability to accurately perceive depth so as to move around work space safely and to appropriately make contact with Veterans. This position requires potentially long periods of continued walking standing stooping sitting bending pulling and pushing. Transferring patients and objects may be required exerting up to 35 pounds of force occasionally and/or up to 15 pounds of force to frequently move objects. Mechanical inpatient lifts are provided. The nurse may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations operative/invasive procedures or home settings. The nurse may occasionally be exposed to patients who are combative secondary to delirium dementia or psychiatric disorders. The nurse must be a mature flexible sensible individual capable of working effectively in stressful situations able to shift priorities based on patient needs. This includes but is not limited to the ability to work independently ability to recognize situations beyond function/knowledge/training and request appropriate assistance. Must complete annual Employee Health requirements such as annual TB screening or testing as a condition of employment. The nurse who provides care in home settings or other off-site locations may be required to drive and/or ride in GSA-vehicles.
