The Registered Nurse Womens Health Quality Coordinator position is located within Ambulatory Care Services at the Phoenix VA Healthcare System (PVAHCS) 32nd Street Clinic Phoenix AZ.
The Registered Nurse demonstrates proficiency in practice based on conscious and deliberate planning. Demonstrates integration of biopsychosocial concepts cognitive skills and technically competent practice while providing care to clients. Delivers fundamental knowledge-based care to the program while developing technical skills. Assesses plans implements and evaluates delivery of care based on population specific components. Collaborates with members of the interdisciplinary team and leading to process improvement initiatives. Acts as a liaison for the coordination of care focused on patient education self-management and customer satisfaction throughout the continuum of care. Follows procedures per established policies and guidelines. Executes comprehensive planning for Womens Health (WH) issues that improves the overall quality of care provided to women Veterans and achieves program goals and outcomes. Core elements of performance include knowledge and active participation in the unit/program and/or service programs.
The Womens Health Quality Coordinator (WHQC) Registered Nurse (RN) executes position responsibilities that demonstrate expert leadership experience and creative approaches in providing complex solutions. The WHQC functions as a liaison and is responsible for planning and coordinating womens health preventative care services in alignment with the Women Veterans Program (WVP) national directives and local facility level policy and procedures. Performs a wide range of clinical administrative program and technical functions including data input and analysis to ensure performance measures exceed benchmarks. Facilitates the coordination of preventative care services with necessary staff to ensure timely and appropriate provision of services across all levels and sites of care. Provides training to promote compliance with processes integral to providing quality care. Utilizes databases and tracking tools for surveillance of quality indicators and outcomes of collaboration with facility leadership and service line stakeholders identifies service needs and contributes to problem resolution due to ability to analyze interpret and evaluate data to determine outcomes and changes. Provides consultation and collaboration through the participation within organizational committees. Responsible for the documented outcomes at the program service and/or facility level. Participates in activities and strategies to sustain evidence-based-practice and continuously evaluates the program collecting and submitting facility performance measures.
