The Registered Nurse provides skilled nursing care case management and hospice care to individuals facing advancing age frailty and serious illness. Responsibilities include patient assessments medication administration treatment planning and collaborating with an interdisciplinary team. Strong communication and collaboration with patients providers and team members are essential.

Required Skills & Certifications:
- ACLS BLS NIH PALS certifications
- Active Registered Nurse license in the state of working
- Clinical Assessment Medication Administration and Treatment Planning skills
- Minimum two years of hospital home health or hospice care experience
- Interdisciplinary Team Collaboration skills
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
- Bachelors degree in Nursing or related field
- Drivers license car insurance and automobile for field work
