Job Summary
A Transitional Care Nurse Practitioner is responsible for coordinating and managing patient care during transitions between care settings such as from hospital to home or post-acute facilities. This role focuses on reducing readmissions by providing comprehensive assessments medication reconciliation patient and caregiver education and timely follow-up care. The nurse practitioner collaborates closely with physicians care management and community providers to ensure continuity of care. Strong clinical judgment communication skills and a patient-centered approach are essential to improving outcomes and supporting high-quality transitions of care.
Qualifications :
Education
- Masters Degree (Required)
Certification / Licensure / Registration
- Certified Registered Nurse Practitioner (Required)
Experience and Skills
- Primary Care or Transitional Care NP Experience Preferred
