Position Overview:
Mental Health Workers (MHWs) at Residential Transitions Inc. provide high-quality person-centered support to individuals residing in our Community Residential Settings (CRS). MHWs assist individuals with daily living skills community integration symptom management behavioral support and overall quality of life. This role requires strong judgment professionalism and the ability to respond to both routine and crisis situations in alignment with 245D standards and agency expectations. Because many of the individuals we support have complex behavioral and mental health needs this position requires prior experience working with high behaviors crisis intervention de-escalation and following formal behavioral support protocols.

Role Responsibilities:
Direct Support & Client Care
  • Provide person-centered support that aligns with each individuals goals preferences and care plans.
  • Assist with daily living tasks such as hygiene cooking cleaning medication administration (after training) transportation and community activities.
  • Implement and follow Behavioral Support Plans PSTPs Safety Plans and other clinical or behavioral guidelines.
  • Maintain a safe structured and therapeutic environment.
Crisis Management & Behavioral Support
  • Utilize approved crisis intervention strategies to respond to escalations high-risk behaviors or mental health emergencies.
  • Document all behavioral incidents using required reporting systems (BIRFs GERs T-Logs).
  • Follow EUMR requirements including immediate notifications documentation and adherence to agency protocols.
  • Support individuals using trauma-informed respectful and rights-affirming practices.
Documentation & Compliance
  • Complete all required documentation in compliance with 245D RTI policy and licensing standards.
  • Report changes in behavior symptoms medical issues and program needs promptly to management.
  • Assist with medication administration and tracking per RTIs Medication Administration Program.
Team Collaboration
  • Communicate effectively with Program Managers Regional Directors nursing staff PSS clinicians and other support partners.
  • Participate in staff meetings trainings and supervisory check-ins.
  • Support a collaborative consistent approach to service delivery and program structure.

Required Qualification and Experience:
The Mental Health Worker shall be qualified under Minnesota law as follows:
  • High School Diploma or GED required; post-secondary coursework in psychology social work behavioral health or related fields preferred.
  • 12 years of experience working with individuals who display high behavioral needs aggression or significant mental health symptoms.
  • Experience with crisis management and de-escalation techniques (CPI Mandt or equivalent).
  • Experience working with EUMR protocols documentation and expectations.
  • Experience understanding implementing and following Behavioral Support Plans Positive Support Transition Plans (PSTPs) and related behavior protocols.
  • Valid drivers license acceptable motor vehicle record and proof of insurance (if transport is required).
  • Ability to pass a DHS background study.
  • Strong communication teamwork and problem-solving skills.
  • Clinical or behavioral services background encouraged but not required.
Preferred Qualification and Experience:
  • Experience working in a licensed 245D or Mental Health residential program.
  • Training or background in trauma-informed care positive supports ASD/ID support or mental health crisis response.
  • Ability to remain calm and effectively intervene during escalations.
  • Experience with documentation systems (Therap MARs BIRFs GERs etc.).

Physical & Mental Requirements:
Employees in this role must be able to:
  • Walk stand sit bend climb stairs use a ladder and perform crisis prevention/intervention techniques
  • Use hands for writing typing data entry and medication dispensing
  • Lift up to 40 lbs. and occasionally exert physical effort during duties
  • Maintain vision and hearing necessary for reading charts operating office equipment using a computer and driving a vehicle
Work Environment
  • CRS homes operate 24/7 and require reliable consistent staffing.
  • This position may involve exposure to high-behavior situations physical aggression or mental health crises.
  • Training provided in crisis intervention medication administration positive supports EUMR and 245D compliance.
Compensation:$22-28 per hour
