The Role
This role which is based in Darwin reports to the Service Delivery Manager ICSS the Care Support Planner assumes the pivotal responsibility of evaluating carers needs guiding them through registration and assessment procedures and subsequently orchestrating their connection to essential services all in alignment with service delivery guidelines. Additionally they evaluate carers eligibility for various supports offered through the Carer Gateway encompassing Emergency Respite Carer Directed Packages Carer Counselling Carer Coaching and Local Peer facilitated groups.
