Social Worker (BSW/MSW)
UnityPoint Health Fort Dodge Emergency Department
Shift: 40 hours per week. Monday-Friday no holidays or weekends!
Social worker to support individuals identified in high utilization of the Emergency Department and Inpatient Units. This social worker will provide an assessment of patient emotional physical and mental needs. Social worker will provide community care coordination and act as a community resource liaison and referral source for patients and families.

Responsibilities
Identify high utilizers of the Emergency Department coordinating with the Emergency Department Medical Director to care plan for identified patients.
Engage with patients when available in the department.
Document patient care plans in the electronic medical record as identified in the program pilot which can be visualized across UnityPoint affiliates.
Interviews patients families community agencies to provide assessment and care coordination.
Adheres to the Social Work Code of Ethics.
Maintains effective communication/working relationships with co-workers leadership and other hospital staff. Behave in manner consistent with FOCUS values.
Participates in educational activities performance improvement activities hospital committees and other related activities.
Demonstrates good verbal and written communication skills with patients family members and healthcare team members.
Qualifications
Education:
Master Degree in Social Work preferred from a school accredited by the Council on Social Work Education.
Experience:
One-year previous social work experience in a health care setting required.
License(s)/Certification(s):
Valid drivers license when driving any vehicle for work-related reasons.
Must possesses an Iowa Social Work License (LMSW or LISW) or obtain within 6 months of hire.
Knowledge/Skills/Abilities:
Effective communication skills. Knowledge and experience in good interviewing techniques. Ability to understand medical terminology and communicate effectively with persons of diverse backgrounds age disabilities and educational levels required. Must demonstrate knowledge skills and abilities to provide services to the infant child adolescent adult and geriatric population.
