POSITION SUMMARY:
Full Time
Shift: 1st Shift (Monday Friday)
Compensation: $19.55 and up (Based on years of experience)
Responsible for assisting the Social Service team in assessment intervention development and care planning of the psychosocial emotional and behavioral needs and well-being of an assigned resident caseload. The Social Work Assistant collaborates with the assigned Social Worker to meet the residents care needs works to maximize the residents independence and enhance the residents quality of life to the extent possible. The Social Work Assistant is also responsible for supporting the Admission Department as well as the residents and their responsible parties prior to admission and throughout the admission process.
JOB DUTIES:
QUALIFICATIONS:
Minimum Qualifications: High School diploma or GED. Associate degree in Social Work Human Services Gerontology Psychology or related field preferred. Caring and outgoing individual with the ability to multitask.
Minimum Experience: One-year supervised experience interacting with older adults and families. One-year supervised experience in long-term care healthcare social services admissions case management or a closely related setting preferred.
