$30.70 - $46.05Step into a role where your clinical insight empathy and problemsolving skills come together to support patients during some of their most vulnerable moments. As a Medical Social Worker you will provide essential psychosocial care guide patients and families through complex decisions and collaborate across the interdisciplinary team to ensure safe effective and compassionate care throughout the hospital stay and beyond. Youll serve as both advocate and educatorhelping patients cope navigate resources and transition confidently to the next phase of care.
Provide psychosocial assessments counseling crisis intervention and information/referral services to patients and families on a consultation basis.
Complete comprehensive psychosocial assessments to support posthospital care planning for designated patients and their support systems.
Assist patients in coping with stress related to hospitalization disability chronic illness or terminal diagnoses.
Collaborate with the Clinical Care Management team to evaluate and recommend the appropriate level of care or care environment across the continuum.
Serve as a resource to hospital staff and physicians regarding emotional social and psychosocial components of illness and their impact on support systems.
Provide ageappropriate education on community resources and posthospital options; make referrals to community agencies as needed.
Report suspected child or adult abuse neglect or exploitation and serve as liaison with the Department of Social Services during evaluation/investigation.
Respond to requests for afterhours consultative services as needed.
Promote optimal clinical and resource outcomes by supporting lengthofstay management reimbursement goals throughput and efficient use of resources.
Act as a central information and education resource for the interdisciplinary team; identify system variances and contribute to organizationwide problemsolving and quality improvement.
Support discharge planning and transitions ensuring continuity of care and reducing avoidable delays.
Masters degree in Social Work (MSW) required.
LCSW preferred; applicable state certification preferred.
Adherence to the National Association of Social Workers (NASW) Code of Ethics required.
At least 1 year of experience in a hospital or healthrelated setting preferred.
Familiarity with or proficiency in data management tools is preferred.
Must meet physical requirements including ability to work in fastpaced environments walk extensively and perform documentation for prolonged periods.
