Job Summary

QMHP Responsibilities:

As the QMHP your key responsibilities will involve the following:

  • Responsible for providing skill-building training in the home or community: one-on-one skills sessions at clients home at the office or in the community relating to goals and objectives using evidence-based practices and curriculum.
  • Provides support and crisis/emergency services for the child/adult/family through face-to-face contact phone contact or availability by an on-call system (all time spent on crisis assessment and planning crisis linkage and follow-up and stabilization must be documented as crisis hours)
  • Maintains all client documentation in the electronic record and writes progress notes that adhere to the Texas Administrative Codes requirements.
  • Utilize practical listening skills to establish rapport with the client family members and other individuals close to the client; provide emotional support and encouragement and engage family members in the service process.
  • Maintains records by regulatory/agency standards and enters data clearly accurately and timely. Also promptly submit documentation needed to track output/outcome measures. May be asked to assist in record review or qualitative review processes.
  • Responsible for keeping the family and team members informed on progress toward achieving Service Plan goals
  • Assisted clients with daily living tasks such as behavior management counseling services budget management independent transportation assistance time management skills and other tasks to promote independent living.
  • Collaborates with court/probation tribal/community agencies families and other natural support systems to meet clients needs and develop strong working relationships with community partners.
  • Assist clients in practicing effective communication problem-solving and anger management techniques daily.
  • Participate in weekly and monthly staff meetings company programs and professional development training as required.
