The Role
- Location: Remote (some travel may be required within the region)
- Two positions available:
- Permanent full-time 38 hours per week.
- Max Term 12 months full-time 38 hours per week.
- Remuneration: Social Community Home Care and Disability Services Industry Award 2010 Level 4. $44.58 per hour
An opportunity has arisen for the role of Support Coordinator within our Community Services division. As a Support Coordinator you will be committed to ensuring that peoples individualised support requirements are fulfilled with a strong focus on seeing people move towards personal goals. The successful candidate will enjoy working in a flexible and constantly changing environment where time management and attention to detail are key. You will need the initiative to work and solve problems unsupervised with a strong focus on reaching set goals and targets.
You will be responsible for supporting NDIS eligible participants with the management of their NDIS plan and budget. You will assist a team of Case Coordinators and other Support Managers to plan implement and evaluate a range of support services which meet participants outcomes. This role operates within a billable hours framework and you will need to ensure strict compliance with all reporting and auditing requirements.
Essential Criteria:
- Demonstrated experience in service coordination or practical experience within the disability sector
- Strong communication skills - written and verbal;
- Ability to liaise effectively with participants customers community groups employees external agencies government departments and other stakeholders
- Valid Working With Children Check for paid employment (or willingness to obtain)
- Knowledge of and commitment to child safe standards and mandatory reporting requirements.
- Willingness to obtain a valid NDIS Workers Check
- Valid NSW Driver Licence (minimum P2)
