Responsibilities
- Deliver high-quality evidence-based advanced clinical care to patients promoting their overall health and well-being.
- Perform comprehensive clinical assessments including taking patient histories conducting physical examinations and interpreting diagnostic tests.
- Develop implement and evaluate individual patient care plans in collaboration with the multidisciplinary healthcare team.
- Diagnose treat and manage acute and chronic medical conditions autonomously while consulting with other team members when necessary.
- Act as a clinical leader and mentor offering guidance advice and professional development opportunities to team members including junior staff and students.
- Contribute to the development and implementation of care pathways protocols and other quality improvement initiatives.
- Ensure adherence to legal ethical and professional standards in line with the organizations policies and values.
- Engage in collaborative decision-making with patients families and caregivers ensuring care decisions align with their preferences and needs.
- Participate in clinical audits research and education to contribute to continuous improvement in practice.
- Promote inclusivity and cultural competence in patient care demonstrating sensitivity to diverse backgrounds and experiences.
Qualifications
- Required:
- Masters degree in Advanced Clinical Practice Nursing or a related healthcare field.
- Active and unrestricted nursing license with relevant advanced practice certifications.
- Extensive clinical experience including autonomous decision-making and management of complex patient cases.
- Strong clinical assessment and diagnostic skills.
- Knowledge of evidence-based practices and healthcare policies.
- Excellent communication teamwork and leadership abilities.
- Dedication to equity and inclusion in healthcare practice.
- Preferred:
- Experience working in multidisciplinary teams.
- Expertise in a specialized area of practice (e.g. geriatrics urgent care or mental health).
- Previous involvement in quality improvement initiatives.
Day-to-Day
- Start each day with interdisciplinary team huddles to review patient care priorities and collaborate on treatment plans.
- Conduct detailed patient consultations physical exams and follow-ups both in-person and via telehealth if applicable.
- Utilize advanced diagnostic tools and techniques to aid in decision-making and treatment planning for both acute and chronic conditions.
- Precept and mentor other clinicians providing educational opportunities and fostering growth within the team.
- Collaborate with hospital departments and community resources to ensure continuity of care and support for patients and their families.
- Document all assessments plans treatments and outcomes in the electronic health record system promptly and accurately.
- Take part in ongoing training sessions and professional development to stay current in advancements in clinical practice.
- Actively participate in quality improvement projects to enhance patient safety care delivery and accessibility.
