Job Summary
The Licensed Practical Nurse (LPN) provides direct patient care in a rehabilitation or skilled nursing setting supporting patient recovery and long-term care needs while maintaining high standards of safety documentation and communication.
Key Responsibilities
Deliver nursing care under the supervision of a Registered Nurse or physician
Monitor patient condition and report changes promptly
Administer medications and treatments as ordered
Assist with patient assessments care planning and daily activities
Maintain accurate and timely clinical documentation
Communicate effectively with interdisciplinary healthcare teams patients and families
Follow facility policies procedures and infection control standards
Requirements
Active LPN license (state valid)
Minimum 1 year of recent LPN experience (Rehabilitation / Long-Term Care preferred)
Current BLS/CPR certification
Strong clinical skills communication abilities and documentation proficiency
