About the Role
You will provide care coordination primarily to First Nations clients with health related goals whilst working within the Healthcare for the Homeless Program at SVHM.
Undertake holistic comprehensive assessments in collaboration with the client carer and/or family using a self-management approach when appropriate
Collaborative care planning with clients with chronic health conditions and complex needs in the context of their health cognition mental status functional ability psychosocial cultural environmental and economic needs
Facilitation of client generated goals that are realistic time limited and based on addressing health and wellbeing needs
Work collaboratively with clients in the community by providing outreach support
Classifications for this position will range from:
Registered Nurse 4A ($118149) per annum.
Allied Health Grade 2 Year1-4 ($90236 - $103724) per annum.
Your Contribution
A health professional with qualifications relevant to ALERT Care Coordination (Social Work/Occupational Therapy/Nursing) minimum 4 years experience in relevant acute/community setting
Eligible for registration/membership with AHPRA or Discipline Specific Association (where applicable).
Current National criminal history check or willing to obtain

ALERT aims to improve health outcomes for people who either frequently present or who are at risk of presenting to hospital because of chronic disease or complex medical and / or psychosocial issues.
The key target groups for ALERT include patients with complex needs who fall under one or more of the key risk factors; substance use mental health issues homelessness disability and complex aged care issues.
