Key Responsibilities
Collaborate to provide management and delivery of patient care plans
Support patients in Wexford and surrounding areas
Consistently meet critical deadlines for charting
Self-manage time and schedule efficiently
Demonstrate experience with tablets mobile phones and EMR software

Schedule and Shift Details Saturday & Sunday 8 AM - 8 PM

Travel Local travel

Qualifications

Required

Associate degree or higher
Hospice (2 years)
1 years of experience in ANY of the following:
- Electronic Medical Record (EMR)
- Other EMR
- TruChart EHR
Valid Licensed Practical Nurse (LPN) in Pennsylvania (PA)
Valid Regular Driver License (DL)
Are you okay with the required travel for this position
