Requirement description :

A Self-Query NPDB report is mandatory at the time of presentation. The Self-Query NPDB report must be dated within 30 days of the presentation date. - REQUIRED
Board Certified - REQUIRED
BLS/ACLS - REQUIRED
OH license (will accept those willing to obtain) - REQUIRED
