Job Summary We are seeking a dedicated Speech-Language Pathologist (SLP) to join a Skilled Nursing Facility (SNF) for a 13-week contract assignment. The SLP will be responsible for evaluating diagnosing and treating patients with communication cognitive-linguistic and swallowing disorders. This role requires strong clinical skills compassion and the ability to work collaboratively within an interdisciplinary team.
Key Responsibilities -
Conduct evaluations and develop individualized treatment plans for residents.

Provide therapy for speech language swallowing and cognitive impairments.

Document assessments progress notes and care plans accurately and in a timely manner.

Collaborate with nursing staff rehabilitation team members and physicians to optimize patient outcomes.

Educate patients families and caregivers on treatment goals and strategies.

Participate in team meetings discharge planning and interdisciplinary discussions.
Qualifications -
Active and current state SLP license (must be in good standing).

Minimum experience: Less than 1 year required (newer clinicians encouraged to apply).

Strong communication and interpersonal skills.
