Job Summary

Job Summary (List Format):

- Assess diagnose and treat speech language cognitive-communication and swallowing disorders in geriatric patients in a subacute/long-term skilled nursing facility (SNF) setting.
- Conduct comprehensive evaluations within 24 hours of physician referral; formulate functional therapy diagnoses and prognoses.
- Develop and implement individualized evidence-based treatment plans with measurable goals; adjust interventions based on patient progress.
- Provide direct therapy for communication and swallowing disorders including compensatory strategies cognitive-communication interventions and group therapy as needed.
- Accurately and promptly document all evaluations treatment notes progress updates physician reports discharge summaries billing and plan of care logs.
- Educate residents families and nursing/CNA staff on communication techniques safe swallowing and home exercise programs.
- Collaborate with interdisciplinary team members (nursing physicians PT/OT dietary social work) to optimize patient care and participate in discharge planning.
- Support quality improvement initiatives and ensure regulatory compliance (Medicare/Medicaid documentation PDPM Title 22 CMS).
- If supervising SLPAs or therapy aides adhere to California supervision regulations and complete required CEUs.
- Maintain current California SLP licensure (temporary RPE accepted) BLS/CPR certification and preferably ASHA CCC-SLP.
- Demonstrate strong assessment treatment planning communication organization and interpersonal skills.
- Manage caseloads that include both short-term rehab and long-term care residents; meet productivity standards (80-90% billable time).
- Adapt to fluctuating workloads and build positive relationships with nursing staff.
- Work in a collaborative multidisciplinary environment focused on enhancing resident quality of life and functional independence.
- Contribute to a supportive and professionally growth-oriented workplace in Californias growing rehabilitative care industry.
