What Youll Do
-
Provide skilled physical therapy services to home health patients based on individualized plans of care
-
Evaluate patients develop treatment plans and modify care based on progress and functional goals
-
Deliver therapeutic exercises gait training wound care and functional mobility interventions
-
Educate patients and families on home exercise programs safety and use of equipment
-
Collaborate closely with physicians case managers and the multidisciplinary care team
-
Accurately document evaluations progress notes and discharge summaries in a timely manner
-
Ensure compliance with Medicare safety and home health regulatory requirements
-
Support continuity of care and participate in team meetings as needed
What Were Looking For
-
Graduate of an accredited Physical Therapy program (Bachelors degree or higher)
-
Active Oklahoma Physical Therapy license
-
CPR certification (or ability to obtain within 30 days of hire)

Strong communication and documentation skills
-
Valid Oklahoma drivers license reliable transportation and auto insurance
-
Ability to work independently in home settings and manage a varied caseload
-
Comfort working with pediatric adult and geriatric populations
Physical Requirements
-
Ability to lift transfer and assist patients (up to or exceeding 100 lbs. as needed)

Ability to stand walk bend and stoop throughout the workday

Capacity to work in home environments and manage occasionally stressful situations
