Job Summary

Location: New York NY 10016
Contract Duration: 14 Weeks
Schedule: MondayFriday 9:00 AM 5:00 PM
Hours: 35 hours/week (1-hour lunch)
Pay Rate: $55$60 per hour
Position Overview
Navitas Healthcare LLC is seeking an experienced Licensed Master Social Worker (LMSW) to support inpatient medical services within a hospital setting. The ideal candidate will have prior hospital-based experience and be comfortable working in a fast-paced interdisciplinary healthcare environment particularly across medicine surgery (med/surg) and rehabilitation units.
Key Responsibilities
Provide comprehensive social work services to patients in inpatient medicine surgery and rehabilitation units
Conduct psychosocial assessments and develop individualized care plans
Collaborate with physicians nurses case managers and interdisciplinary care teams
Support patients and families with discharge planning and coordination of community resources
Maintain accurate and timely documentation in compliance with hospital policies and regulatory standards
Ensure patient confidentiality and adherence to all organizational policies and procedures
Perform additional duties as assigned
Required Skills & Experience
Minimum of 1 year of experience as a Licensed Master Social Worker (LMSW)
Hospital-based experience required
Experience working in inpatient med/surg and rehabilitation settings
Strong assessment communication and documentation skills
Ability to function effectively within a multidisciplinary healthcare team
Education Requirements
Masters Degree in Social Work (required)
Licensure Requirements
Active Licensed Master Social Worker (LMSW) license issued by New York State Education Department (NYSED)
