responses to community concerns.
RESPONSIBILITIES:
The Community Health Program Manager is responsible for the strategic development implementation
and management of all Health & Wellness community programming within the Health Equity Department.
This includes oversight of Produce Rx nutrition education food insecurity initiatives and public
health focused wraparound services delivered through Community Health Workers. The Program Manager
ensures that all programs are community-centered culturally responsive and aligned with Adelante
Mujeres mission to advance health equity for Latina women and their families. This role supervises
program staff strengthens partnerships leads program planning and evaluation and contributes to
the overall strategic direction of the Health Equity Department. The Program Manager reports to the
Director of Health Equity Programs.
Provide critical leadership and strategic direction for Health & Wellness programs with a
primary focus on Produce Rx nutrition education food security initiatives and Community
Health/public health services.
Monitor trends community needs and public health issues related to nutrition food security
and broader community health initiatives and lead a coordinated response to meet changing needs
Cultivate and maintain relationships with community partners healthcare organizations public
agencies and other stakeholders.
Supervise program staff to foster a collaborative high-quality team environment that values
trust open communication and flexibility in a fast-paced work setting.
Regularly assess program needs and resources and develop the program budget accordingly.
Complete and submit grant reports as required and ensure that all documentation and reports
are accurate and completed in a timely manner
Ensure confidential and accurate use of internal systems including HIPAA compliance to
maintain participant information and program records.

partners

QUALIFICATIONS:

Required:
Strong commitment to the mission of the organization environmental sustainability employee
well-being and support for immigrant and BIPOC communities
Bachelors degree in social science or a related field OR Community Health Worker (CHW)
certification with a minimum of 2 years of experience leading a CHW team
Language fluency in English and Spanish including the ability to read write and speak
Minimum of three years of program management including strategic planning and program
operations
Excellent verbal and written communication
Minimum 1 year experience with evaluation of services data collection documentation grant
reporting and rationale for services.
Minimum 2-year experience in partnership building and cross-organization collaborations
Minimum 2 year proven ability to develop working relationships across a variety of internal
and external stakeholders
Proficiency with Microsoft Office365 Zoom and CRM systems
Preferred:
Minimum 1 year experience working and organizing individuals with barriers to accessing
essential services
Minimum 1 year budget management experience with the ability to develop monitor and adjust
program budgets
Ability to travel locally and nationally to meetings and conferences
Preferred experience facilitating and coaching using popular education methods
Obtain Food Handlers card within 90 days of hire

SALARY & BENEFITS:
Salary range: $68K-$71K depending on qualifications and experience
Classification Full-time salary position
Telework: up to 1 day a week depending on the departments needs.
