The Calgary West Central Primary Care Network (CWC PCN) is a non-profit organization committed to a healthier future. We work alongside more than 490 family doctors to provide support for continuous health care.
The Primary Care Registered Nurse (PCRN) is a key member of the interprofessional healthcare team. Working to full scope of practice with a holistic approach the PCRN assesses educates supports refers case manages and evaluates patients in regards to their health promotion needs mental health needs and chronic disease management.
This role is primarily a work-from-home position with occasional in-person responsibilities; therefore the successful candidate must be located within the Calgary Zone or the surrounding area (e.g. Cochrane Chestermere Airdrie etc.).
This position is a temporary maternity leave contract and the expected contract end date is February 26th 2027.
Responsibilities
Clinical expertise
- Counsel and guide patients on symptom management health maintenance and rehabilitation strategies as well as risk factors and lifestyle changes and involve them in decision-making and self-management.
- Provide and coordinate screening for health issues such as diabetes hypertension cholesterol cardiovascular health mental health and cognitive function.
- Work collaboratively with the interprofessional team to identify plan implement and measure patient-centred health improvement goals.
- Implement and facilitate groups for health promotion.
Patient-centred care
- Complete comprehensive and person-centred assessments develop care plans synthesizing health assessment information and use critical thinking and clinical reasoning skills to identify health concerns and sustainable realistic interventions in partnership with the patient.
- Use HealthChange Methodology to provide person-centred care and promote health literacy shared decision-making behaviour change and self-management to support adherence to evidenced-based recommendations for improved health and quality of life outcomes.
- Assist in setting up health promotion programs and patient self-management programs.
- Direct patients to the care they need from the appropriate health professional or program.
Quality improvement
- Collaborate with other teams to identify patient groups and target goals to improve patient care.
- Participate in the identification of new service requirements and addresses deficiencies in current services for the benefit of patients.
- Offer training and support for other patient care team members and assist in their professional development.
RequirementsEducation: A bachelors degree in nursing is required. Education in mental health and chronic disease management is a strong asset. Professional education related to quality improvement (QI) in the workplace is an asset.
Experience: Experience in broad clinical practice including specific experience in mental health chronic disease such as diabetes hypertension weight management/fitness respiratory (asthma COPD) management and womens health is required. Clinical experience in a primary care setting is an asset. Previous experience in patient teaching or group facilitation is an asset. Experience with successful QI initiatives in the workplace is an asset.
Registration: Active registration with the College of Registered Nurses of Alberta (CRNA) in good standing is required.
Certifications: Canadian Diabetes Educator (CDE) certification or willingness and eligibility to complete is an asset. Additional certifications in chronic disease management (e.g. Certified Respiratory Educator) are an asset.
Technical: Proficiency in the Microsoft Office Suite (Word Excel Outlook PowerPoint Teams) and EMR systems. Experience in and/or willingness to learn collaboration software and virtual care platforms.
CPR: Current BLS Level C CPR/AED issued by the Heart and Stroke Foundation.
Backgroundcheck:A clear Criminal Background check with Vulnerable Sector check is required to start.
