At Rehab and Nursing Center Cedar our teams specialize in long-term care fostering deep connections with our patients their families and friends. Committed to enhancing the quality of life for our residents we maintain staffing levels and quality scores that exceed regulatory standards. Our facility designed with the word home in mind opened its doors in May 2024. It features four beautiful internal courtyards including one exclusively for team members. Our new community dining room is a vibrant space frequented by residents for meals and activities complemented by an onsite art room and dialysis den.

Rehab and Nursing Center Cedar is dedicated to providing a homelike atmosphere for our long-term care residents. Each resident room is equipped with modern furnishings new technology and a personal living space including a window to the outdoors a private restroom with a shower and amenities to support their well-being. Our facility was meticulously designed to create a welcoming environment where residents can thrive comfortably in this new place they now call home.
Scope of work
Registered Nurse (RN)
Delivers nursing care responsibly and accountably as described professionally by the American Nurses Association (ANA) and legally by the State of Michigan. Demonstrates competency through the critical thinking model known as the nursing process. This includes assessment diagnosis outcomes identification planning implementation and evaluation. Upholds the standards of professional performance as described by the ANA. This includes ethical practice culturally congruent practice communication collaboration leadership education evidence-based practice and research quality of practice professional practice evaluation resource utilization and environmental health. Strives to achieve optimal outcomes.
Licensed Practical Nurse (LPN)
Under the direction of the RN/Mid-level Provider/Physician and in collaboration with other members of the health care team contributes to the assessment planning implementation and evaluation of patient care. Informs patients and families about treatments and medications being received equipment being utilized and other related activities as appropriate in coordination with the RN/Mid-level Provider/Physician. Documents care provided via computer/written patient record in a manner consistent with guidelines.
