Job Summary

We are seeking a part-time Patient Advocate position available for a skilled and compassionate professional to provide community resource support to Hope Clinic patients at the Westland clinic location. The Patient Advocate works to assist current Hope Clinic patients with health insurance enrollment food and medication assistance basic needs and community resource referrals.


We anticipate 18-20 hours per week for this part-time position. Weekly schedule is Tuesday and Friday 8:30 - 4:30 with an additional/flexible 2 hours per week for meetings or community engagement as directed. Position offers paid vacation and sick time in addition to a generous amount of paid time off including the week between Christmas and New Years.


Required Education and Experience

  • BA preferred
  • Demonstrated human service or related client resource advocacy work
  • Experience navigating healthcare enrollment systems and general healthcare issues strongly preferred
  • Experience working with diverse client populations with a variety of needs


Required Competencies

  • Strong communication and client engagement skills
  • Knowledge of community resources in Wayne County (in particular the Westland/western Wayne County area)
  • Ability to calmly engage clients in challenging situations
  • Strong computer proficiency
  • Collaborative and team-oriented
  • Ability to work in a small shared-space clinical environment


Primary Responsibilities

  • Healthcare navigation: Identifying and assisting Hope patients who may be eligible for health plan coverage such as Healthy MI to enroll in the appropriate program and seek services at other clinical sites
  • Completing on-line enrollment applications or referrals for the Healthy MI Plan and other coverage plans
  • Completing client needs assessments
  • General resource advocacy: assisting Hope Clinic clients navigate community resources and providing referrals
  • Teaching capacity-building skills to patients to complete forms anticipate re-enrollment deadlines and utilize health care appropriately
  • Developing clear easy-to-read materials including but not limited to checklists of items needed for insurance and Washtenaw Health Plan enrollment and a simple handout on options for low-cost or free medication
