As a Transition to Community Peer Support Specialist Team Lead you will provide support and supervision to the Peer Support staff and provide therapy to their consumers. The purpose of the Peer Support Specialist Transition to Community Living service is to enhance the consumers ability to function and achieve their goals while residing in an independent living/community setting. The ratio of supervisor to CPSS is 1 supervisor to 8 Peers. The Peers will have a maximum caseload of 15 clients. The individuals being served are diagnosed with a Mental Health and/or Substance Use addition the individuals will be living in Permanent Supportive Housing and participating in the Transition to Community Living program. The intent of the service is to increase an individuals ability to live successfully in the community by maintaining tenancy.

Position Highlights:

  • Provide individual family and group therapy using evidence-based practices aligned with client needs and diagnoses.

  • Supervise and support Certified Peer Support Specialists including administrative oversight and quality assurance of services.

  • Collaborate on recovery-oriented person-centered planning and ensure timely assessments and documentation.

  • Facilitate therapeutic and skill-building groups and ensure access to community resources as needed.

  • Maintain clinical documentation including treatment plans progress notes and required reporting.

Compensation:$57807 - $67135

Essential Functions:

  • Supervises a designated group of QMHP-level staff providing performance oversight mentoring and support.
  • Responsible for managing staff productivity utilization documentation compliance quality assurance and adherence to timecard and PTO policies.
  • Participates in personnel-related decision-making including scheduling performance feedback and corrective actions as needed.
  • Ensures that staff maintain compliance with clinical and operational policies and standards.

Operational Leadership:

  • Independently manages referral workflows and case assignments reviewing documentation and making case decisions as appropriate.
  • Coordinates with community partners and stakeholders to ensure successful service delivery
  • Leads weekly Quality Assurance (QA) reviews to ensure documentation and service delivery meet regulatory and internal standards

Training and Development:

  • Provides onboarding and orientation for new hires including development of training materials and performance support during the onboarding process.
  • Oversees ongoing staff training and ensures compliance with mandatory training requirements (e.g. CPR CPI).
  • Serves as subject matter resource on workflows and service delivery expectations.

Administrative and Program Support:

  • Assists the Site Manager with facilities-related issues and contributes to operational planning.
  • Participates in leadership and operational meetings to help shape and implement site-level strategies and improvements.
  • Supports marketing and outreach efforts in coordination with program leadership.
  • Administers and tracks temporary pay stipends and incentive programs in accordance with policy.
  • Manage the referral tracker/ case assignments
  • Authorization management (initial and continued stays) as needed.


Qualifications

Education: Masters degree inPsychology Social Work or a related human services field is required.

Certification: Licensed as a LCSW/A LCMHC/A in NC.

Experience:Minimum two (2) years of proven direct care and/or management experience in the health and human service field. Knowledge in Business Administration and/or Quality Assurance preferred.
