Job Summary

Physical Therapist In-Home Outpatient (Older Adult Population)


Occupational Exposure: Category I.
Department: Rehabilitation.
Reports To: Director of Rehabilitation

Position Summary:


The Physical Therapist provides outpatient rehabilitation services in an in-home setting for the older adult population. Services are delivered in patients private residences or within independent living facilities allowing therapy to occur in the most functional environmentthe patients home.

The Physical Therapist assesses patient needs and develops implements and delivers individualized physical therapy programs to facilitate rehabilitation. This role includes identifying the need for referrals to other disciplines to address comprehensive patient needs implementing treatment plans designed to restore and enhance performance and educating patients and caregivers to promote independence productivity and overall health. This position offers a flexible schedule and varied work settings.

  1. Salary range: $40 - $55/ Hour

  2. Guaranteed35 hours in a week

  3. 5 days a week

  4. No weekend rotation

Key Responsibilities

  • Supervise Physical Therapy Assistants temporary licensees students and support personnel in accordance with state licensure requirements and professional standards.
  • Maintain adherence to patient treatment schedules clinical documentation standards billing requirements and work-related meetings.
  • Participate in patient care conferences staffing meetings and interdisciplinary collaboration.
  • Make referrals to other disciplines and agencies to support comprehensive patient care.
  • Consult with rehabilitation team members to select appropriate therapeutic interventions based on patient needs and capabilities.
  • Adapt plans of care based on patient progress and individual needs.
  • Promote patient safety and maintain a hazard-free treatment environment.
  • Order clean maintain and report repair needs for equipment and supplies.
  • Deliver patient-centered care with compassion respect and professionalism.
  • Clinical Duties
  • Screen examine and evaluate patients through history systems review and appropriate tests and measures.
  • Interpret findings to establish diagnoses identify impairments precautions and contraindications and determine prognosis and treatment timelines.
  • Develop measurable treatment goals in collaboration with patients and caregivers.
  • Implement and document physical therapy interventions including but not limited to:.
  • Therapeutic exercise (aerobic conditioning balance coordination agility posture flexibility gait training neuromotor training relaxation strength power and endurance).
  • Functional training.
  • Manual therapy.
  • Airway clearance techniques.
  • Integumentary repair and protection.
  • Electrotherapeutic physical agent and mechanical modalities.
  • Health and wellness education.
  • Chronic disease management education.
  • Fall risk assessment and fall-prevention interventions.
  • Home and community case management.
  • Design fit and train patients in the use of adaptive equipment including orthotics and prosthetics.
  • Select activities appropriate to the patients physical and cognitive abilities.
  • Instruct patients in mobility skills and assess progress and rehabilitative potential.
  • Perform routine re-evaluations to modify or progress treatment plans.
  • Conduct home and community mobility assessments and provide safety recommendations.
  • Perform other related duties as assigned.
  • Education & Licensure Requirements.
  • Valid Physical Therapy license in the state(s) of practice or proof of eligibility.

One of the following:

  • Masters degree in Physical Therapy.
  • Masters degree in a Physical Therapy-related field plus a Bachelors degree in Physical Therapy.
  • Bachelors degree in Physical Therapy with five (5) years of progressively responsible experience.
  • International degrees must be evaluated for U.S. equivalency.
  • Valid drivers license and reliable transportation to patient residences.

  • Ability to lift push pull and carry up to 50 pounds.
  • Regular reaching handling fingering fine manipulation bending stooping kneeling crouching crawling twisting balancing and stair climbing.

  • Primarily indoor patient-home settings with limited outdoor exposure.
  • Exposure to noise environmental hazards uneven surfaces and automotive equipment.
  • Occasional use of personal protective equipment (gowns gloves masks).
