Position: Integrated Care Nurse Practitioner (Hybrid)
Schedule: MondayFriday Flexible hours
Hybrid Role: Guaranteed 1 day per week remote
Travel: Statewide travel 80% of the time (4 days/week)
Occasional overnight travel (company paid)
Compensation: Base salary 15% quarterly bonus

Overview
We are seeking Nurse Practitioners or Physician Assistants who enjoy variety and flexibility. This role supports patients with chronic and complex conditions including kidney disease through a mix of statewide in-home visits and telehealth. Youll work as part of an integrated care team alongside a Registered Nurse and Social Worker.

Key Responsibilities

Conduct comprehensive assessments and annual wellness visits (home & virtual)
Develop and manage individualized treatment plans
Prescribe medications and order diagnostics
Coordinate care with physicians specialists and interdisciplinary team members
Educate patients and families on disease management and preventive care
Complete timely accurate clinical documentation

Requirements

Active Nurse Practitioner or Physician Assistant license and board certification
Active DEA license
2 years of direct patient care experience
Ability to practice independently
Willingness to travel statewide (overnight travel paid)
Valid drivers license and reliable transportation
