Hybrid Role: 12 days telehealth (remote) 34 days in patients homes
Compensation: Base salary 15% quarterly bonus

Overview
We are seeking Nurse Practitioners or Physician Assistants to join a home-based care team serving patients with chronic and complex conditions including kidney disease. Youll work alongside a Registered Nurse and Social Worker to deliver personalized patient-centered care through a mix of in-home visits and telehealth.

Key Responsibilities

Perform comprehensive assessments and annual wellness visits (home & virtual)
Develop and manage individualized treatment plans
Prescribe medications and order diagnostics
Coordinate care with physicians specialists and care team members
Educate patients and families on disease management and preventive care
Maintain accurate timely clinical documentation

Requirements

Active NP or PA license and board certification
Active DEA license
2 years of direct patient care experience
Ability to practice independently
Valid drivers license and reliable transportation
