Schedule: MondayFriday 8:00 AM5:00 PM
Hybrid Role: 12 days telehealth (remote) 34 days in patients homes
Average Volume: 34 patient visits per day
Compensation: Base salary 15% bonus

Overview
We are seeking compassionate Nurse Practitioners and Physician Assistants to join a home-based Palliative Care team focused on improving quality of life for patients with serious and chronic illness including kidney disease. Youll work as part of an integrated care team alongside a Registered Nurse and Social Worker delivering patient-centered evidence-based care through a blend of in-home visits and telehealth.

Key Responsibilities

Conduct comprehensive assessments and annual wellness visits (home and virtual)
Develop implement and manage individualized treatment plans
Prescribe medications and order diagnostic testing
Coordinate care with PCPs specialists and ancillary services
Educate patients and families on disease management and preventive care
Maintain accurate timely clinical documentation
Participate in interdisciplinary care team meetings
Follow up and evaluate patient progress to improve outcomes

Requirements

Active unrestricted NP or PA license
Board certified (NP: ANCC/AANP PA: NCCPA)
Active DEA license
2 years of direct patient care experience
Experience with annual wellness visits (home or virtual preferred)
Ability to practice autonomously
Valid drivers license reliable transportation and insurance
Managed Care / IPA / Health Plan experience preferred
