How youll contribute

APharmacistwho excels in this role:

• Coordinate and provide pharmaceutical care for patients collaborating with interprofessional team to optimize safe effective medication use.
• Review and verify orders manage drug therapy protocols educate staff/patients and support quality and regulatory standards.
• Manage drug therapy protocols as part of an interdisciplinary team.
• Review interpret and evaluate medication orders to identify prevent and resolve drug-related problems.
• Provide drug information and education to medical staff healthcare professionals patients and pharmacy students.
• Document interventions and outcomes for drug therapy quality improvement.
• Verify medication orders for accuracy/completeness prior to entry.
• Check filled medications prepared by technicians.
• Maintain proficiency with pharmacy automated systems and software.
• Manage patients in an outpatient anticoagulation clinic including point-of-care testing result evaluation dosing and scheduling.
• Participates in performance improvement and required continuing education.

What were looking for
Applicants should have a doctorate degree. Additional requirements include:

• Current license with the state Board of Pharmacy.

• BLS required.

• ACLS preferred.
