Description

The Medical Coder 1 abstracts clinical information from a variety of medical records charts and documents and assigns appropriate ICD-9 and/or CPT-4 codes to patient records according to established procedures. Familiar with standard concepts practices and procedures within a particular field. Relies on instructions and pre-established guidelines to perform the functions of the job. May work with coding databases and confirms DRG assignments. This position relies on established guidelines to accomplish tasks and works under close supervision.

  1. Coding/Abstracting
    1. Correct coding assignment of DRG; Discharge Disposition; Querying Physician and Principal Procedure and Diagnosis.
    2. Determines the appropriate sequencing of diseases diagnoses and surgeries.
  2. Quality/Performance
    1. Coding accuracy rate of 97% of INPT SDS ER and Outpatient Diagnostics.
    2. Responsible for montoring bill hold on a daily basis.
    3. Bill Hold days should not exceed 4 days.
    4. Fosters an organizational climate that supports and promotes effective performance improvement efforts.
    5. Ensures that appropriate priority is given to provide high quality care by ensuring guidelines are followed for core measures through concurrent chart review and follow-up with appropriate healthcare provider.
    6. The ability to determine if a chart is incomplete versus complete.
    7. Ability to understand what forms need to be maintained in each account (INPTSDS and AMB).
    8. Notifies approperate persons of admissions and discharges per protocol.
    9. Communicates accurate information with payor and physician to insure coverage for services/care provided.
    10. Immediately notifies physicians and Business Office if potential non-coverage issues evolve. Actively works to maintain or gain coverage for patient.
    11. The ability to continuously provide quality improvement techniques for the electronic medical record system.
    12. Utilizes time to accomplish work in an efficient manner.
  3. Other Duties as Assigned
    1. Adheres to hospital policy on the usage of Paid Time Off and Extended Illness Time.
    2. When requested adjusts personal schedule to meet department/unit needs.
    3. Reports to work on time and if necessary reports tardiness to appropriate person.
    4. Maintains a professional appearance according to job requirements at all times.
    5. Employee shall conform to regulatory customer and organizational requirements.
    6. Attended all required mandatory educational sessions.
    7. Utilizes Personal Protective Equipment (PPE) in the appropriate manner for optimal personal protection. Operates equipment and performs job-related duties in a safe manner. Responds appropriately to fire safety and disaster situations.
    8. Attended all required mandatory educational personal responsibility for ongoing continuing education and professional development.
    9. Adheres to infection control practices. Has completed required TB skin tests.


Qualifications

Experience - RHIT/RHIA or 1 year of acute care coding experience and/or ER E&M and Drug admin charging experience; CCS substitutes for 1 year of acute care coding experience; Associates or Bachelors degree in Allied Health or Health Information Systems can substitute for minimum years of experience.

Education - High School Diploma or equivalent
