Erie Family Health Centers a nationally recognized top workplace with 13 sites in Chicago and suburbs!The Community Health RN Team Lead provides leadership support and oversight to nursing staff and is Responsible of implementing the day-to-day operations and patient flow for nursing care at the assigned clinical site.

Main Duties & Responsibilities
- Triage assessment process of walk-in patients and telephone calls.
- Delivers health education related to medication diet treatment of diagnosis health promotion prevention and exit health teaching.
- Administers immunizations applicable contraception refills and medications as ordered by provider.
- Provides guidance and support to nursing staff at assigned sites.
- Provides for regular nursing team meetings and 1:1 supervisory meeting with direct reports.
- Other duties as required.
QualificationsEducation
- BSN required masters in public health or related field desirable
Skills and Knowledge
Required:
- Current BLS for Healthcare Provider CPR certification
Preferred:
- Minimum three years of experience in community health nursing or ambulatory care preferred
- Supervisory experience preferred
