Supervise home care registered nurses and certified nursing assistants. Coordinate clinical care of assigned home care patients with support services team coordinator.
ESSENTIAL DUTIES & RESPONSIBILITIES:
Partner with support services team coordinator to assure quality patient/family care
Assure adequate staffing to provide quality services to patients/families
Assure outpatient services are appropriate and coordinated with business services and support services team coordinator
Be available daily to provide direction to team assuring appropriate interventions
Review time records for accuracy and approve
Assure team members have adequate supplies equipment and physical space
Maintain current knowledge and competency in providing care to terminally ill patients and their families
Assist with entering Hospice referrals into electronic medical record as needed
Coordinate the determination of appropriateness of referrals with hospice physicians
Coordinate patient assignment of primary nurse and certified nursing assistant
Communicate with on-call team members and patient care unit staff on regular basis
Supervise certified nursing assistant assignments
Assure timely and appropriate utilization review of patients
Coordinate pharmacy medical equipment and other contracted services and monitor compliance
Coordinate delivery of care with primary team and attending physician
Be available to receive telephone calls from staff physicians health care facilities and families to coordinate services with primary team
Perform duties of primary nurse and patient care unit nurse as needed
Coordinate continuous care services
Assist with the maintenance of medical records assuring accuracy completeness and compliance with all regulations and standards
Supervise patient medical supplies inventory
Assure monthly nursing audits are completed
Assure compliance with CNA assignments
Communicate all clinical information to Chief Nursing Officer and Chief Clinical Support Services Officer as appropriate
Assure 24-hour on-call services and be available for on-call services as needed
Teach electronic documentation to new nursing staff
Interview hire and supervise all nursing services team staff complete all required performance evaluations and clinical competencies
Coordinate orientation for new nurses and certified nursing assistants
Assure all nursing services staff promote the philosophy of hospice as directed by the chief executive officer
Assure all home care nursing services staff adhere to personnel policy
Coordinate continuing education for registered nurses and certified nursing assistant staff
Complete reports and other statistical data as requested
Keep abreast of all regulations and communicate information to nurses and certified nursing assistants
Rotate administrative back up call as scheduled
Assess staffing patterns on an on-going basis and apprise chief nursing officer of needs
Other duties as assigned

MINIMUM QUALIFICATIONS:
Education:
Bachelors degree in nursing or comparable degree in healthcare or health sciences required
Masters degree in nursing preferred
Experience:
Strong background in healthcare particularly in hospice and palliative care settings. Prior direct patient care experience in hospice and end of life care is preferred
Healthcare supervisory experience
Licensures & Certifications: (if applicable):
Hospice certification preferred
PHYSICAL DEMANDS:
Visual acuity to read written documentation and computer tasks
Ability to sit extended periods of time and perform administrative tasks
Ability to stand bend and lift an average of 30 pounds
Manual dexterity with the ability to manipulate office equipment and keyboard for extended periods of time
SPECIAL REQUIREMENTS:
Must have an automobile with required insurance coverage and state drivers license
